Hockey Moves Risk, Liability, and Photo Release Agreement

IMPORTANT – PLEASE READ CAREFULLY

This document contains the terms and conditions under which your child(ren) may participate in Hockey Moves hockey programs (the "Activities"). You are being provided this document in full via the link. Prior to enrolling your child(ren) in any Hockey Moves program, you will be required on a separate page to indicate your acceptance of these terms by checking an acceptance box. Your electronic acceptance (via the check box) is intended to constitute your agreement to the terms set forth below and is legally binding.

1. Assumption of Risk

I understand and acknowledge that participation in Hockey Moves hockey programs, including camps, practices, games, and related activities ("Activities"), involves inherent risks. These risks include, but are not limited to, the possibility of physical injury, accidents, or other unforeseen events that may result in serious injury or death. I further understand that hockey involves fast-paced movement, physical contact, and the use of equipment that may pose hazards.

I voluntarily assume all risks associated with my child(ren)'s participation in these Activities.

2. Waiver and Release of Liability

In consideration of permitting my child(ren) to participate in the Activities, I, on behalf of myself, my child(ren), and our respective heirs, assigns, and legal representatives, hereby fully and forever release, waive, discharge, and covenant not to sue Hockey Moves, its officers, directors, employees, agents, volunteers, and representatives (collectively, the "Released Parties") from any and all liability, claims, or causes of action, whether arising out of negligence or otherwise, for any loss, damage, or injury (including death) that may occur to my child(ren) or to any property in connection with participation in the Activities.

3. Indemnification

I agree to indemnify and hold harmless the Released Parties from any and all claims, demands, or liabilities, including legal fees and costs, arising out of or in any way connected with my child(ren)'s participation in the Activities.

4. Photo and Video Release

I grant Hockey Moves the irrevocable right and permission to record, photograph, and/or videotape my child(ren) during the Activities. I consent to the use of such images and recordings for promotional, marketing, and educational purposes in any medium, now known or hereafter developed, without compensation. I waive any right to inspect or approve the finished product wherein my child(ren)'s likeness appears.

5. Electronic Acceptance

By accessing this document via the provided link, and subsequently checking the acceptance box on the designated page, you acknowledge that you have read, understood, and agree to all of the terms and conditions set forth in this Agreement. Your electronic acceptance via the check box is intended to serve as your legally binding agreement to the terms of this document.

6. Governing Law and Severability

This Agreement shall be governed by and construed in accordance with the laws of the Province of Ontario. If any portion of this Agreement is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

By enrolling your child in a Hockey Moves program, you acknowledge and agree to these policies.
